I really don't like this entry:
F0 - system.ini: Shell=true.exe

These seem to be missing something:
F2 - REG:system.ini: Shell=
F2 - REG:system.ini: UserInit=

On top of that you have no firewall or anti-virus protection

I recommend you follow these Viruses/Spyware/Malware, preliminary removal instructions and post back in this thread with the requested logs. There should be at least 3.

1)AVG log
2)Combofix log
3)Hijackthis log (Step 15)
