Moniter randomly looses HDMI Signal then freezes

So this has been going on for a few weeks now. I'll be in a game and my monitor will go black and then an alert pops up saying HDMI no signal. I have to restart my computer and when I do that all my fans go on full rpm. If I try turning the monitor back on while the fans are going at full RPM it will show the same message but once the fans have stopped I have to restart the computer once more and it works fine. This happens every 4-5 hours sometimes sooner. It seems completely random and sometimes has happened when im just watching you tube. Thanks in advance for any help you guys can give me.

I would start by monitoring temps on the graphics card - HWInfo works for me.

I would also check Event Viewer.
