Network switch - can fiber port swap around internally?

We have an Ethernet network that extends with a multi mode fiber optic cable. on the two ends we use an HP ProCuve 2124 network switch, and a RUBYtech network switch. They are basically just used as media converters.

Recently I have been called out to work, and when I arrive, the personnel tell me that there was a power failure and after the power was restored, the comm's was just lost.

After some investigation, I found that if I swap the patch lead ports around, the comm's come back online. The first time this happened, I figured another technician might have swapped it by accident, the second time I thought maybe it was the operators that swapped it around, and now before I accuse anyone, I just want to know, is it possible for the switch to swap the ports internally? Can any hardware failure cause such an error to occur?
