Final update:
After quite a lot of testing my final conclusion was this.
My issues were down to a few things both hardware & software related:
Bad stick of RAM installed in slot 1
Acronis True Image Home 2009 - backup imaging software
VMWare Server
Hope this saves anyone else 6 months trying to work out

Interesting - especially about Acronis. Was this because it was unable to handle locked files, perhaps?
