USB flash drive - unreadable, can't format, cannot access

Symptoms in Windows 7/8

1) I Can see drive in "My Computer"
2) Double click on drive in my computer and it says : "Please insert disc into drive H:"
3) Right click and select format and it says: "There is no disc in drive H. Insert a disc, and then try again"

plz provide any solution

Try removing or reformatting the partition from Disk Management. If you can't, see if you can delete the partition and create a new one.
