Affected adapters were sold with iPhones in the US, Canada, Mexico, Japan and several Latin American countries. Apparently, this is the result of a fairly poor design approach used to save money. No injuries have been reported, but Apple is urging iPhone 3G customers to bring their adapters to an Apple store for an exchange and in the meantime charge via USB through their computers.
