Memory could not be read

We are using Windows NT 4.0 on a dedicated server, and we are getting the following message after re-booting, and attempting to export a file to an LVT film recorder:

The instruction at "0x780011ce" referenced memory at "0x003e0000". The memory could not be "read".

We have replace the ram with no result. Also, cannot open Photoshop. We ran MemTest for as long as possible, but apparently no report was generated.

Any suggestions? Can anyone translate the numbers or codes above to tell us what part of the memory is the problem?
