Ati video card need help w/id

I have an ati agp video card that I have not used in so long that I canot find any of my info or cd for. Below is some info that may help you all to be able to help me.
The large chip near the fan says:
ati rage theather

bbe21
0050ss
taiwan

from top to botom of the back access panel i have:
s-vid in
catv in
s-vid out
vga monitor connection
p/n 1027670100039245

I have more info :
on the flip side of the board there is a ati radeon graphics tm stamp (white)
2000 ati techonlogies inc. -copyright stencled into the green board,
