Audio is lowered when using headphones

Since I downloaded the Windows 10 upgrade a few months ago, every time I plug in my headphones to listen to anything on my pc, for a second the sound is loud but then within a few seconds it lowers to barely audible even though I have the sound up 100%. I've tried searching to find a fix for this but so far everything I tried does not work. It is not the headphones because I use them in my iPad and they work fine and they worked fine before I upgraded to Windows 10. Any help with this issue is appreciated.

Is Windows 10 fully updated? There have been many improvements over the last month or two
