dll initialisation failed message when shutting down

I keep getting the message box "dll initialisation failed" everytime I go to shut down windows. There is further text underneath but I don't get chance to read it before windows starts shutting down.

Try to hit the pause key when you re-create the error and see if you can get more of the info.
