Help with monitor

MY monitor is all fuzzy and says warning! signal out of range, how do i sort this? when i start pc up it's still fuzzy i dont get how ??

what kind of monitor LCD or CRT? Make and model would be nice... It is possible the your video card has died or you have messed with the video settings. Can you bare to give us a little more information?
