External Hard Drive Connecting and Immediately Disconnecting

I have a 5tb external hardrive that I am trying to connect to my pc. When I connect it, it will give the plug and play notification. At about 30 seconds or so after it disconnects, not giving me an opportunity to view or partition the hard drive. I have tried multiple usb ports, and even bought a new wire to see if that was the issue. I tried to run a chkdsk via admin command prompt, and it did not help me at all. Any other suggestions would be greatly appreciated.

What is the OS?
Is the HDD partitioned? GPT or MBR?
