As a guest mentioned above, 'carrier exclusive' deals also did much hurt to Nokia+MS platform, I reckon it is one way of helping your competition by restricting access to your own product.

Secondly and probably IMO just as important (if not more), Microsoft never understood the mobile game to start with, hence, they tried to implement desktop model of licensing fee on windows phone, whereas, everyone else could easily get a 'free' OS (I.e. Android), slap it on their phone and sell it. I raised this issue with couple of people linked with MS on twitter since 2011/12 I think, and yet they never had any logical answer to this. Now fast forward to 2014, they've finally made windows phone free for any OEM, and look what is happening, even 1+ is now talking about making a version of windows phone of their top end android phone. IMHO many OEMs probably don't like the situation they are in right now, I.e. totally reliant on Google for everything, which is why having free windows phone can be an interesting option now.
