Is my graphics card dying?

So I have a ATI Radeon 9250 Graphics card. The game I mostly play is madden 2005. Always works perfect. But the other day the game started getting REALLY SLOW, to the point where it was tough to even get out of it. I went inside the computer, touched the video card, and I could've cooked an egg on it.(dont run a fan on it). I ran crap cleaner, defragged, and ran all my virus/spyware scans. Played it again and it worked. Well this morning, i went to play it again, and it was horribly slow again. Went inside the computer, and it was even hotter. Put a fan on it, and it cooled down, but everything is still real real slow. Do you think that it overheated and now it is damaged? Thanks for the help!
