Frankly speaking, this is one of the most infected computer, I've ever seen.
I assume, you've noticed all those infected files, I included in original OTLPE script.
It may be actually much easier to boot from OTLPE CD, back up your data to USB stick and reinstall Windows.
This computer may be simply beyond repair.
