Confused by power supply requirements

Sorry if I'm posting in the wrong forum. I need to aquire a replacement ac adapter for my Canon 8600F scanner. My problem is the label on the faulty adapter appears to have sets of specifications. How do I determine the correct specs for the replacement. I'm at a loss. Below is crude drawing of the label. Can anyone help?

Take that to Radio shack and they should fix you up.

Pretty sure that:

Input is 100v AC @ 0.5A 50/60Hz. (this is the mains electric side)
Output is 12v DC @ 1.5A 50/60Hz. (this is the device side, that plugs into scanner)

You can either order a genuine or pattern power unit, or just take it to an electronics place and get a multi-voltage power unit with the right connector.
