Voodoo 3 2000 PCI version, can't forget the first one. Especially when 3DFX makes awesome looking boxes for the Voodoo series.

First game I loaded on it was Half-Life 1. Good times.

Also, still have a laptop with an ATi Rage Pro 8mb which can play Deus Ex (with tweaked drivers), Baldur's Gate II, Planescape, Starcraft....
 
First 3D card I had was a Diamond Monster 3D Voodo 4MB card. Paired it up with the 1MB onboard video I had and was blown away by how awesome Star Wars Jedi Knight: Dark Forces II looked cranked up to 640x480.
That card lasted me for years. Still have fond memories of it.
 
You're all still babies by the looks of it, my first card was one for monochrome graphics on a PC XT in the mid '80s.

While SiS 6326 let me down in gaming department many times I have to admit that it sparked my interest on GPUs. As a kid I had the impression any 'PC' can play any 'PC' game (just like gaming consoles do) until one day I got a pop up 'Your graphics card doesn't support Hardware T&L' while trying to run a game.

So my GPU enthusiasm began with 'What the hell is Hardware T&L??'
 
If you don't count what came in my old Compaq Portable One...then I think the first genuine "PC" one was an old Trident.

My first PC came with an Intel i740. I played a lot of unreal tournament, freespace 2, and crimson skies on it.
Soon though I upgraded to a GeForce 2 MX (32 Mb), playing pretty much the same games plus MechWarrior 3, but with the graphics options cranked up.
For my first home-built PC I had a GeForce 6600GT, and was playing Unreal Tournament 2004.
After about four years the fan died and the card fried, so I went with an 8800GT (512 Mb). I played a lot of Command & Conquer 3 and the Kane's Wrath expansion by then.
I built another PC to upgrade to a Core 2 Duo, and I put a GTX260 in that build. The old computer with the 8800GT became my wife's PC in a new chassis. I started playing Bad Company 2 with a clan about that time (I had generally shunned multiplayer before that, first due to having dial-up until 2006 and then due to the fact that I didn't like getting routinely slaughtered on M-rated games by 10 year old pottymouths who played more hours per week than I worked).
Now I have a GTX480, with the 8800GT running PhysX. I played a lot of Battlefield 3, until I suddenly got burned out on it a couple of months ago. Now I am mainly playing Freespace Open (the Freespace upgrade, along with lots of stand-alones such as Diaspora, and mods and new campaigns), so I have sort of come full circle, playing basically the same game that I was playing on my i740 but at much higher resolution and with much more eye candy.
The GTX260 is now in my son's PC.
 
80x286 CPU, don't remember the video board, it was probably an S3.

Then S3 trio with Voodoo 1, 4 MB VRAM.

Then Voodoo Banshee, Riva TNT, Geforce2 Ti, Geforce 4 TI 4200 (runs fine to this day), Geforce 6600 GT, Geforce 8600 GT, Geforce 9800 GTX+, GTX 260, Radeon 6850 (black screens, had to return it, permanently ending my short relationship with ATI), GTX 460 768, GTX 460 1024 (my current card).
 
Most of my early use of graphics was funded by work ( Chips and Technologies EGA, NEC µPD7220 etc.). First graphics bought was for my then fiancée's system - an Orchid Fahrenheit 1280 (S3 911), followed by a succession of ATi Mach cards.
Wouldn't try listing all I've owned, but since joining TS I've owned HD 4890 (+ CF), GTX 280 (+ SLI), HD 5770's, HD 5850's (XFX Black Ed.'s and Sapphire Toxic 2G), GTX 580 (+ SLI), and my present GTX 670 (Palit Jetstream).

I forget exactly what was in my Viglen P90 before I dropped my first 3D card in (though it might have been a Diamond Stealth 64), but my first 3D card was the Orchid Righteous Voodoo 3D (Voodoo1). Had a Voodoo 3 2000 or 3000 after that

My first card was an S3 Trio32. Replaced a few years later with a Riva TNT2, followed by a Radeon 9200SE, and then later with a GeForce 6600 (non-GT), that lasted me quite a while, until I invested in a pair of X1950XTs. These were replaced later with a 8800GTS 512 (a downgrade period where I had to sell the old PC to fund part of my education costs), which was eventually replaced by my current (aging) HD 5850.

I was on a pretty regular AMD-NVIDIA tick-tock cycle. ;)
 
The first video card I ever purchased was a brand new MacMagic 8MB 3Dfx Voodoo1 card by Village Tronic. It was for a Apple Macintosh PowerMac 6500/300. We got the computer in 1997 and I believe we bought this card in 1998. I still have the video card and the box it came in, all of which is in excellent condition. The computer on the other hand was trashed a long time ago.
The games I played on this card were Tomb Raider I and II, Myth I and II, Quake I and II, Future Cop: LAPD, Duke Nukem 3D, Dark Vengeance, and Unreal. Unreal was the last game that was played on this card because in the year 2000 we got a new computer. Fun times!
 
First card I purchaes of a friend was a 3dfx voodoo 3. (High five pacostrano)

Switched to a nVidia TNT2 because the system it was in overheated because of it.

Then a MX440. I think I finished Half-life 2 on that thing :p

ATI x800Pro, which still runs in a friends system 'till this day. That Sapphire card is indestructible and can run Crysis (sort of)

nVidia 8800GTS512, been baked twice I believe... as all 8000 cards of nVidia are :p

Now I'm running on a 560Ti SLI setup. Maybe looking for something else when the 700 series arrive.
