Transferring files from Windows to iPhones

Out of curiosity, how does an iPhone user transfer media from Windows to their iPhones? Must they install a proprietary app from Apple so they can do so?

Music and Movies are imported to iTunes and then the iPhone can import them by a Lightning cable connection to the PC or by Wifi if so configured.
