Crash while gaming

So pretty much any modern game will cause my game to freeze, my monitor will stop receiving from my computer, then a grid of boxes pops up that keep changing color. During the entire time the sound from my game keeps going. I had this problem a while back so I bought a 200mm fan and the issue stopped, so I don't know what it is now.

RAM: 4g
Graphics Card: ATI Radeon HD 4800

Sounds like a video card problem. Have you updated your drivers? Do you know if the card is overheating?

If you have more than one PCIe slot on your motherboard try swapping the card to another slot.
