quick format and create boot disk not working properly in XP

I'm trying to upgrade the bios to VLB44. A while back I installed a radeon 9600 and have had intermittent problems ever since. Thought I had installed this bios but don't have access to the onboard chip function. Tried to format a floppy but it won't do a quick format. Will create a bootable but when I try to boot with it get Disk I/O error. any help appreciated!
