I need a driver for a creative i/o ultra ata ide controller card driver sy-pci45004

I've got the cd, but I can't get it to work properly, so I need to get an updated driver. I don't know where to look. Please help if you can.
 
None of them at the site seem to match the model number sy-pci45004. They could be generic drivers and will work with more than one card, but if so, I don't know which one will work.
 
Here is what I did. I did a simple google search of the model number and found the chipset type from here: http://www.newegg.com/product/product.aspx?Item=N82E16816124025. Note it says ITE8212 chipset.

I found the manufacturer's website by clicking on Newegg's manufacturer's product page link further down that same page. That takes you to here: http://www.syba.com/index.php?controller=Product&action=Info&Id=1037

A little searching on Syba's website brought me to the page I linked to in my first post. The chipset number 8212 is the key number. It is the second card shown.

You didn't list your operating system so I can't get more specific but do you see the driver in question?
